Registered Nurse – Complex Care

Single de About the Role Are you a registered nurse with the clinical expertise and ambition to help build something meaningful? Bluebird Care Welwyn & Hatfield is looking for a Registered General Nurse (RGN) to take on a pivotal role, helping shape, grow and deliver safe, high-quality complex care services across our community. We are at an exciting stage of our development. As part of our TDDI registration, we are bringing a nurse into the team to help us grow our complex care provision. This means you will be joining at the ground level, with real influence over how our clinical services are built. What the Role Looks Like Initially you will be working 16 hours per week, split across 2 or 3 days. During this period, while our complex care packages are being developed, your time will be focused on: Supporting business development, including building and maintaining relationships with hospitals, discharge teams, GPs, case managers and local community services Raising awareness of our complex care offering across the local area Helping us prepare clinically so we are ready to deliver from day one when packages come in As complex care packages are secured, your hours and responsibilities will grow. Over time the role will include: Leading complex care delivery in the community Conducting clinical assessments and developing tailored, person-centred care plans Providing clinical supervision, training and competency assessments for care staff Overseeing risk management, safeguarding and governance compliance Ensuring CQC and infection control standards are consistently met Supporting staff development and contributing to service improvement Why Join Us? Care Coordinator

Job Description: We have an exciting opportunity for an enthusiastic person to join our award-winning care team, which was rated as Outstanding by the CQC at our last inspection. We are expanding in both Amber Valley and Derbyshire Dales and are looking for the right person to co-ordinate our care business from our office in Belper, managing the rotas and ensuring compliance is up to date. This role offers a great opportunity in social care including to be part of a strong, supportive team, providing highly sought after home-care and support services in the area. The majority of our customers are privately-funded with our average visit time of about an hour or more. Preference will be given to applicants with rostering or social care or healthcare experience. What we offer Our initial salary package is up to £28,000, based on experience and upon successful completion of probation after 6 months; 20 days holiday plus Bank Holidays Pension scheme; Training and development opportunities. Job Details In this role, reporting to the Registered Care Manager, and working with the Customer Care Manager, you will coordinate our customer support visits by allocating appropriate members of the care team to customer calls. You will do this using a computerised, real-time rostering system. In addition to this, you will liaise with the customers and care team to manage any changes to visits and rosters in an ever-changing environment. Following full training, you will assist in answering telephone enquiries, and making sure all enquiries and changes are recorded accurately and delegated to the correct team member. In this role, you will keep records current, so accuracy and excellent attention to detail are essential. The ability to problem-solve and think clearly when under pressure is essential for this important role in our company as is your attention to detail. You will use your excellent planning, organisational, communication and IT skills to make sure that each customer receives the care and support that they expect, on time. This involves: Allocating care assistants with the right skills and experience to deliver the best quality care and support to each customer Organising care assistant rotas to maximise continuity of the service and keep efficiency of travel between customer’s homes Responding efficiently to day-to-day changes resulting from changes in the care and support packages Ensuring that all documentation is present and up to date including training and employee documents. The role offers the opportunity for developing positive working relationships with customers, their families and your colleagues in the Bluebird Care team. You will provide a crucial link between each customer and their care assistant. You will often deal with queries from customers about their care and support. This requires a sensitive understanding of customer needs and a supportive and professional telephone manner. The Coordinator must also be clear about the best way to work with Care Assistant schedules to maintain continuity in the delivery of care and build strong relationships with the care team who we rely on to cover the calls. Care Assistants work over weekends and evenings so the post holder must be willing to be part of the on-call team rota. *Full training and support will be provided before the successful applicant is expected to take full responsibility for the tasks listed.
