Our customers should feel comfortable and supported in their own homes, and effective care coordination helps make that possible. You’ll be responsible for planning and maintaining care rotas, managing changes and working closely with our customers, their families, Care Assistants and the wider office team.

Responsibilities
As a Care Coordinator, your key responsibilities will include:
- Planning and maintaining care rotas at least four weeks in advance.
- Matching Care Assistants to customers based on their needs, preferences, availability and location.
- Ensuring sufficient travel time is provided between visits.
- Managing daily rota changes and arranging cover for sickness, holidays, absence and emergencies.
- Coordinating new care packages and implementing changes as customers’ needs develop.
- Ensuring visit times meet customer expectations and service standards.
- Communicating professionally with customers, families and Care Assistants.
- Maintaining accurate electronic records and documenting important communications.
- Responding to coordination concerns and supporting their prompt resolution.
- Working closely with the wider office team to maintain a safe, reliable and person-centred service.
- Participating in the shared out-of-hours on-call rota, in addition to the usual working hours.
Benefits
- Salary of £26,436.80 to £29,500 per year, depending on experience.
- Full-time, Monday to Friday, 8:30am to 5:00pm.

Requirements
- Previous experience in care coordination, homecare or a similar scheduling role is preferred.
- Excellent organisation, communication and problem-solving skills.
- The ability to prioritise and make effective decisions in a busy environment.
- Confidence using electronic systems and maintaining accurate records.
- A calm and professional approach when managing unexpected changes.
- A strong commitment to delivering high-quality, person-centred care.
- Knowledge of care regulations and compliance requirements.
- A full UK driving licence and access to a vehicle.
