Location: Manchester North & Salford (office-based with field visits)

What You’ll Do

  • Responsible for rostering and ensuring all visits are fully and safely covered
  • Visit customers to review person‑centred care and support plans, making updates as required
  • Introduce Care Assistants to customers and keep customers informed of any changes
  • Liaise with GPs, pharmacies, and district nurses to ensure a joined‑up approach
  • Regularly review care notes and escalate/communicate any concerns
  • Provide safe, compassionate care and ensure customers feel informed and supported
  • Create and review customer support plans and risk assessments
  • Support with rota cover where required, and participate in on-call duties.

About You

You share our vision for high‑quality, customer‑focused care and bring:

  • Strong rota management and travel coordination skills
  • NVQ Level 3 in Health & Social Care (or working towards it)
  • Proficiency in administration, staff management, and IT systems
  • Understanding of CQC Essential Standards of Quality and Safety
  • A dynamic, ambitious, and hard‑working approach, with strong attention to detail
  • Excellent verbal and written communication and active listening skills

Essential:

  • Full UK driving licence and access to your own car
  • Minimum 1 year’s experience as a Care Coordinator
  • Effective time management and ability to prioritise
