What You’ll Do
- Responsible for rostering and ensuring all visits are fully and safely covered
- Visit customers to review person‑centred care and support plans, making updates as required
- Introduce Care Assistants to customers and keep customers informed of any changes
- Liaise with GPs, pharmacies, and district nurses to ensure a joined‑up approach
- Regularly review care notes and escalate/communicate any concerns
- Provide safe, compassionate care and ensure customers feel informed and supported
- Create and review customer support plans and risk assessments
- Support with rota cover where required, and participate in on-call duties.
About You
You share our vision for high‑quality, customer‑focused care and bring:
- Strong rota management and travel coordination skills
- NVQ Level 3 in Health & Social Care (or working towards it)
- Proficiency in administration, staff management, and IT systems
- Understanding of CQC Essential Standards of Quality and Safety
- A dynamic, ambitious, and hard‑working approach, with strong attention to detail
- Excellent verbal and written communication and active listening skills
Essential:
- Full UK driving licence and access to your own car
- Minimum 1 year’s experience as a Care Coordinator
- Effective time management and ability to prioritise
