Care Coordinator

We are seeking an organised and proactive Care Coordinator to join our team. This is a fast-paced role where clear, accurate, and timely communication is essential to ensure safe, responsive, and person-centred care.
Salary: £27,000 per annum (40 hours per week)
Hours: Full-time (Part-time considered)
Availability: Must work every other weekend, with 1 in 4 weekends on call
Location: Exeter
You will coordinate schedules, support customers and carers, and act as a key communication link between the office, care staff, customers, families, and external professionals. You will be using internal software to coordinate diaries and planning of Care assistant rounds in the community.
Key Responsibilities
· Coordinate new and existing customer schedules, including templating, allocations, and day-to-day changes
· Communicate all rota and care changes promptly to customers, carers, and NOK
· Cover sickness and unplanned absences while keeping everyone informed
· Manage new care packages, carer introductions, and start dates
· Respond to customer queries, concerns, and minor complaints
· Maintain accurate records, on-call logs, capacity trackers, and incident actions
· Monitor log-in/log-out alerts and follow up as needed
· Liaise with GPs, District Nurses, Social Workers, and other professionals
· Support care staff with availability, holidays, welfare calls, and supervisions
What We’re Looking For
· Excellent verbal and written communication skills
· Ability to communicate calmly and clearly under pressure
· Strong organisation and attention to detail
· Experience in care coordination, scheduling, or a similar role (preferred)
· Confident using care systems and Microsoft 365
· Flexible, reliable, and team-focused
