As the leading Home Care provider, we take pride in our work, and we’re looking for a caring and dedicated individual to join our Bluebird Care Cardiff and The Vale Team as a Live-In Care Coordinator to help us set the standard in our Live-In Care packages.
As a Live-In Care Coordinator, you’ll be the heartbeat of our service, ensuring seamless scheduling and coordination of our outstanding Care Assistants across Cardiff and The Vale. This is more than just a job – it’s a chance to make a real impact while enjoying fantastic rewards.
Key Responsibilities
Live-In Care Duties:
- Scheduling live-in care packages, matching carers to clients based on preferences, skills, and compatibility.
- Creating bespoke care plans and risk assessments for new live-in care clients.
- Completing reviews of care plans and risk assessments in line with CIW regulations.
- Completing weekly handovers with live-in care staff and clients.
- Ensuring completion of Daily Average Agreements and acting on any changes in dependency for each live-in care package.
Care Coordinator Duties
- Scheduling and matching Live-In Care Experts to customers.
- Ensuring smooth and seamless care delivery with full shift coverage.
- Processing new referrals and updating care needs.
- Liaising with the office team to maintain accurate records and reports.
- Supporting our care team, ensuring they have everything they need to succeed.
- Participating in the on-call rota (one day per week and one weekend in four).

Requirements:
- Senior Care Assistant experience with a minimum of 1 year in the care sector
- Previous experience as a Care Coordinator
- NVQ/QCF Level 2 or above in Health & Social Care
- Social Care Wales registration (desirable but not essential)
- Full UK driving licence and access to your own vehicle
- Ability to stay organised and thrive under pressure
