As the leading Home Care provider , we take pride in our work - and we’re looking for a caring and dedicated person to join our Chelmsford team as a Care Coordinator , to help us set the standard. As a Care Coordinator , you’ll help our team grow, support our colleagues, and make a real difference every day. The role includes 20 dedicated hours per week of care coordination, with the option to work additional hours as a Care Expert, up to full-time hours in total. You will ensure customers receive safe, consistent and person-centred care in their own homes, while supporting our Care Experts through efficient scheduling and clear communication. Key Responsibilities Create and maintain efficient rotas, ensuring care is delivered on time and in accordance with customer care plans. Work with the Registered Manager to ensure appropriate staffing levels and skill mix. Arrange cover for staff holidays and absences. Process new customer referrals and changes to care needs promptly. Support Care Experts by monitoring workloads and promoting work–life balance. Match Care Experts to customers based on skills, experience, availability and preferences. Be an active participant in on-call, following training and competency sign-off. Maintain accurate digital and written records, including travel and incident reporting. Ensure confidentiality and adherence to company policies at all times. Work collaboratively with colleagues and external professionals to deliver exceptional homecare services.
